Our practice offers Heart Health Checks for people aged 45 years and above, and Aboriginal and Torres Strait Islander patients aged 30 years and above. This service aims to identify a person’s risk of developing cardiovascular disease and formulate measures in partnership with the patient to reduce their risk.
The Heart Health Check involves an in-depth assessment of your health and risk factors, and health education to reduce risk of developing cardiovascular disease by the nurse and doctor. You will need to have some blood test results within the past year. If you haven’t had any, we will facilitate obtaining a pathology referral for you from your doctor.
